At All Utah Forms, we deeply respect the copyright of content creators and are committed to protecting their rights. We adhere to the principles set forth under the Digital Millennium Copyright Act (DMCA) and are here to assist in resolving copyright disputes quickly and effectively.
Submitting a Copyright Complaint
To file a copyright infringement notice, please provide us with a detailed notice that includes the following information:
- A physical or electronic signature of the copyright owner or a person authorized to act on their behalf.
- Identification of the copyrighted work claimed to have been infringed.
- Identification of the material that is claimed to be infringing or to be the subject of infringing activity, including information sufficient to allow us to locate the material.
- Your contact information, including your address, telephone number, and email address.
- A statement by you that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
- A statement that the information in the notification is accurate, and, under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.

Our Response to Copyright Complaints
Upon receiving a valid copyright complaint, All Utah Forms will take appropriate action, which may include removing or disabling access to the infringing material. We also reserve the right to terminate the access of users who are repeat infringers. Additionally, we will notify the party that posted the allegedly infringing content, providing them with an opportunity to respond.
Counter-Notification
If you receive a notification from All Utah Forms that material made available by you on or through the website has been the subject of a copyright complaint, you may provide a counter-notification. This counter-notification must include your physical or electronic signature, identification of the material that has been removed or to which access has been disabled, and the location at which the material appeared before it was removed or access to it was disabled. Additionally, you must include a statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of a mistake or misidentification of the material to be removed or disabled, and your name, address, and telephone number.
We will then provide the complainant with your counter-notification and inform them that we will replace the removed material or cease disabling access to it in 10 business days. If the complainant does not file an action seeking a court order against you within that time frame, we will replace the removed material or cease disabling access to it.
